At Hermitage Three Chopt, I am responsible for working with families as they navigate making the decision to move their loved one into our community where we serve up to 48 individuals, all of whom live with some form of dementia. This move takes careful and thorough coordination between the future resident's primary care physician and our community. When I am working with someone who utilizes At Home Harmony (AHH) for primary care, I am relieved, knowing the process is going to go smoothly due to the thorough care they give their patients, and their excellent communication between the family, the care provider and nurse at At Home Harmony, and myself. My AHH families always rave about the care and attention they receive from AHH - as if they are receiving concierge care but without the cost. As a chosen provider to facilitate Medicare's GUIDE program, I often recommend families contact AHH to align with this wonderful benefit. The wealth of knowledge and care provided by this team to help families navigate the journey of dementia, whether living at home with care or transitioning into a memory support community, is second to none. Tara Beatty has a heart (and an incredible background) for this work and is an amazing source of support and resource for families. I highly recommend At Home Harmony as a medical provider and GUIDE program participant!
The support offered by GUIDE and At Home Harmony is exceptional. Knowing and understanding how Dementia functions (or doesn’t) for a family member makes a world of difference. Before hearing or joining the program my brothers and I were struggling to understand and deal with our mother’s progression. Of course the path and pain is still extremely tough, but GUIDE has been through very similar situations with other families and freely ( literally) shares that experience and offers physical (through respite care) and emotional relief and support for caregivers. So glad for Tara Beatty and the rest of the GUIDE team for the amazing work they do. Rather than “too good to be true” the GUIDE program is “so good and so true” to the family caregivers of dementia patients for what I would consider the saddest disease for a family to deal with.
I was referred to At Home Harmony by another organization due to my parents' care needs. I am coordinating care for both of my parents who have advanced dementia. It has been a process to find the right combination of care and caregivers. We looked at Memory Care facilities. The problem is that these are Real Estate plays and the care is often secondary to simply collecting large sums of money for varying qualities of departments. Enter At Home Harmony. With the GUIDE program, they provide medical care for my parents at home. This has brought a consistency of care with specialized medical professionals who understand dementia. The Home Health Aids have generally been good, which is what makes At Home Harmony special. The people they have managing them are incredibly responsive and are helping navigate insurance and personnel issues that are inevitable when providing care to dementia patients. It took about 6 months for me to get everything organized on my end, in the interim, my parents have had very good care. There have been hiccups, but they have been handled well. Thanks to At Home Harmony, I have been able to help my parents through a very difficult period and keep them in their home as long as possible.
